I'd guess that your problem is simply clients with misconfigured reverse DNS
connecting to your server.  Try setting "HostnameLookups double" and see if that
makes it go away.  In general, you will want to have "HostnameLookups off" for
performance reasons, however.

In any case, unless you can confirm that this is a bug, it is better discussed
in a user form like the apache users mailing list.
