I can't reproduce it with worker + perl 5.8.4 (using HEAD for mp2, 
httpd-2, apr and apr-util).

>From the stacktrace you posted, it doesn't *look* like an apreq problem 
because the "r" passed to ap_run_handler is the same "r" pointer that's
passed to mpxs_Apache__RequestRec_content_type (so Apache::Request
is handing off the proper request_rec pointer to the base class during
the $req->content_type('text/plain') call).

Any ideas as to the cause?
