I would like to try to pack some of the changes needed for this issue into the coming ALPHA6
release, especially those that affect the public API. While you are working on header iterators
I could give you a helping hand and deal with the low level token parsing. 

How about adding the following methods to the HeaderValueParser / HeaderValueFormatter interfaces?
Does thing go along with your line of thinking?

HeaderValueParser interface: 

String[] parseTokens(CharArrayBuffer buffer,
                                 int indexFrom,
                                 int indexTo)
        throws ParseException
        ;

HeaderValueFormatter interface: 

    CharArrayBuffer formatTokens(CharArrayBuffer buffer,
                                     String[] tokens);
