Attaching patch that has been rebased to master

While investigating potential causes of increased mean time I noticed a small bug where NextState.NO_MORE_VALUES
was being returned from KeyValueHeap#next() inappropriately (didn't cause issues because users
of this state weren't actually using this state, just the Results returns from the call).
This bug has been corrected in this patch but will not provide any performance increase. 

Let's put it through a final QA run to make sure this fix hasn't created any new issues. I
will fill out the release notes shortly
