Here is a trunk patch, which only fixes the exception in the block cache as per above. 

My proposal is to go with these v2 patches for trunk, 0.95 and 0.94, and do a follow up with
the more intrusive approach (passing blockcache and lock containers down from region servers
to store file readers). Suggestions welcome.
