It looks as if the failure cases have hasAllCodeFlags() == false. Not sure if that's a coincidence
or not. In my test cases, codeSpecialHeader (and thus codeFlagsCount) is always zero when
hasAllCodeFlags() is false in ClassBands.parseCodeBands().
