The attached two files (main.patch and test.patch) provide a fix for the BCI-renumbering issue.
They also add code to support  BCI-renumbering of LocalVariableTableAttributes, but leave
it disabled for now since it's currently not renumbering correctly.

This also makes a fix to invokespecial_new_init - it was using the wrong bytecode, which will
cause class verification errors.
