I'm still learning more about the actual security protocol so I'll have to wait until I can
make a good judgement there.  In the meantime is there a reason to alternate taking std::strings
as parameters in the same function both by ptr or ref? 

e.g.
class DigestMD5Authenticator {
...
private:
  static size_t NextToken(const std::string &payload, size_t off,
                          std::string *tok);

I think it would make things more maintainable to pick one and stick with it; references are
typically implemented as pointers so the performance should be the same in either case.
