appendRaw should take:

void appendRaw(byte[] key, int keyOffset, int keyLength,
                 byte[] value, int valueOffset, int valueLength);

so that you can pass in subsets of byte arrays.

The InputStream appender is a *raw* appender. So it breaks the abstraction to pass as non-raw
key with a raw value.

If we support user-specified meta-data, it should be settable until the close method is called.
Since you might want to include summary information about the contents of the file.
