David,

I think the problem was that the hierarchy of handlerCollection to contextHandlerCollection
was not setup properly.   So start was not propogating down. You should not have to call setServer
on anything.

With my changes, I was able to run up the server with the 4 normal contexts.
So I will look at your change, but will probably back it out and replace with mine.
