I believe a "deploy" operation could fail because under the covers it was implemented as "distribute
then start" but we ignored the status of the "distribute" operation, and then something unfortunate
happened with the "start" after a failed distribute.  Now we should print a proper error if
"distribute" fails.

If you find any further conditions that cause a silent failure, please reopen this with more
