I made some small changes from your patch when I checked in and I 
wanted to be sure you had no objections to them.

If I recall correctly the main differences are:
1. I'm still using the ExtendedTransactionManager interface to avoid 
extra use of thread locals where possible
2. I'm still keeping track of times internally using milliseconds 
rather than seconds.
