Ok, so from looking at the logs, here is what I've found:
locator-0: fatal - membership service failure due to loss of quorum
server-1: fatal - membership service failure, ForcedDisconnectException:
member isn't responding to heartbeats
server-2: fatal - locator declares that a network partition has occurred,
ForcedDisconnectException.
client-3: no fatal logs, warn x2 - pool unexpected closed socket on server
connection

If anyone is interested in looking through the log files, I have zipped
them up and attached them. If they are not attached, let me know and we'll
figure out how to work around the apache listserv limitations.
