IMO the first line indicating the dtd point to a wrong URL :
<!DOCTYPE document PUBLIC "-//APACHE//DTD Documentation V1.2//EN" 
"http://forrest.apache.org/dtd/document-v12.dtd">

If the dtd was found, the xml-validator should have given a clearer 
message, but since there is no dtd...
