The original review was only basic, so we may have lost some features.

Items A, B, C are fixed.

However Items D and E still remain in plugin ...internal.dispatcher/resources/stylesheets/html/book-to-menu.xsl
These are fixed in main/webapp/skins/common/xslt/html/book-to-menu.xsl
