Site elements without a label should be ignored, just like they are ignored when displaying
the menu on HTML pages.

But including pages linked to by tabs is probably a good idea.  However if the tab does *not*
have an entry in site.xml (either labelled or not), there's no clear location to put it in
the aggregate PDF.  Currently the site.xml heirarchy is used to organized the PDF.  Probably
just at the end will be fine.

The site.html and site.pdf files automatically generated do not include the content in the
index.xml files (at many levels).

My site.xml references the index files, i.e.,

<site ...

  <index href="index.html"/>


  <module tab="module" href="module">

    <index href="index.html"/>

Without a label, Forrest does not generate a clickable link in the menu; you navigate via
the tabs at the top.
