This exception should not be thrown in normal state.  I checked the code, and its logic didn't
have any problem.  The possible reasons are:

* Messages are being sent from multiple threads at the same time.  (It can be a problem because
there's no synchronization code in SimpleProtocolEncoderOutput)
* User might have implemented his ProtocolEncoder to write *null* to ProtocolEncoderOutput.
 SimpleProtocolEncoderOutput doesn't prohibit for now.
