Regarding the pkfk.patch:

There should not be a PrimaryKey element in element, only in Join.

We are missing the definitions in join, element, key, and value for Index and Unique. These
are specified in xml to allow the user to specify the name of the constraint, not just the
fact that the constraint exists. 

We are missing the references to ForeignKey, Index, and Unique on PersistenceCapable. These
xml elements allow specifying constraints at the class or interface level.
