Good catch on the PreparedStatement issue. In practice, I imagine
that Xplain-Only mode is only likely to be used in special debugging
situations, not in production applications, so it seems uncommon that
an application would routinely be switching it on and off.

But I can see that in many situations it might be easiest for an
application writer to insert the Xplain-Only requests into an existing
application in a way that mimics the scenarios you outline, so we
should try to give these patterns of use reasonable behaviors.

Perhaps I should set the flag in both locations: in the constructor,
and again in open(). I don't think it's an expensive check to make.

I'll update the patch with some additional tests using PreparedStatements.

And thanks for the testing, and for the catch on the eol-style; this
is a new machine and I missed that part.
