Those files are generated by DITA, so I don't know if there's much we can do. For the record,
I tried to generate docs with the newest DITA (1.4.1, whereas the build scripts currently
use 1.1.2.1) and can verify that DOCTYPE and META are still missing in index.html. However,
the toc.html file has a DOCTYPE tag with that version of DITA.
