Thanks Knut for catching that I had only partially removed the implementation.

I suppose another alternative would be to remove the reference from the usage
message (and the test output), but leave the -ca implementation in place.

I'm a little concerned about the utility of a feature for which (apparently) the
community has written neither documentation nor tests. By proposing to
remove the implementation, I was agreeing with John's observation that if
we don't wish to document and test the feature, why do we want to keep the code?
We could always recover the code later from the svn history if we changed our mind.

I think at this point I find that I don't understand what purpose the feature has, but
on the other hand am quite happy to leave the feature if the community feels it
might be useful.

But if we decide to leave it, it seems like we ought to be able to come up with
enough of an example of how to use it, that we could turn that example into
a test and some documentation.

Kim, when you originally reported this issue, you wrote that the -ca option
was "no longer needed and is deprecated"; do you recall what discussions led
to that conclusion?
