After looking this over, I don't know why the changes occured that Kim discovered. 
In my editor, the end </topicref> tags appear appropriately.  When I open the 
ditamap in notepad, I can see that some of the end tags are on separate lines, 
so I have fixed them in notepad and attached a patch and the revised ditamap file.


Please look these over and let me know if it is ready to commit.
