A few more problems with Working with Derby manual

After rerunning the tasks in Working With Derby, I found a few more issues.

1) There's a step-ordering problem: The Introduction tells users to verify that DERBY_HOME
is set, but instructions for setting it don't appear till Activity 1. The instructions for
setting DERBY_HOME should be moved to the Introduction.

2) In step 2 of Activity 1, add a statement that ij commands can be in either uppercase or
lowercase, to prevent confusion when both forms are used.

3) Fix horizontal and vertical spacing in command and code examples.

4) Correct the derby.log output and description.
