More detail: the problem of ignoring whitespace between adjacent tags applies to all adjacent
tags in body text, as near as I can make out. Two examples:

the <ph conref="wwdconrefs.dita#prod/productshortname"></ph> <codeph>ij</codeph>

the <ph conref="wwdconrefs.dita#prod/productshortname"></ph> <varname>URL

In the monohtml and PDF, these come out as "the Derbyij tool" and "the DerbyURL attribute";
"ij" is in code font and "URL attribute" is in italics, but the spaces are lost.

Also, here is a simpler example of the problem with codeblocks. The following results in preserved
line breaks:

<b><userinput>java MyProgram</userinput></b>

program output

But the following, where the spaces are between two tags, does not:

<b><userinput>java MyProgram</userinput></b>

<systemoutput>program output</systemoutput>

Interestingly, using <pre> instead of <codeblock> causes the line breaks to be
