Thanks for the catch Andrew.  I guess I didnt svn add it the first time.  The attached patch
called derby377addendum.diff is an update.  It only includes that missing file.  I assumed
thats what you wanted since the others have already been committed.  Let me know if you wanted
a full patch.

As for the sample output, its already in the first patch zip file.
