DERBY-4253: Document behavior of upgrade=true when db already booted

This change adds some additional documentation to the manual page for
the upgrade=true attribute to clarify the behavior when the database
is already booted in soft upgrade mode: a full upgrade cannot be
performed when the database is already booted; the database must be
shutdown and then re-booted in full upgrade mode.
