I think I'd need to see the full interface and the code for the Exceptions. 

I tried it here and I'm not seeing any duplicate elements.   It might be a namespace issue
where the faults are in different namespaces from the types or something and I'd like to see
all the annotations so I can figure out which namespaces are being used where.
