ResourceInjector passes incorrect resource names

The ResourceInjector passes incorrect resource names to ResourceResolvers. For example, for
the given annotation:

@Resource
private DataSource myDB;

resolveResource() is called with name = myDB and type = javax.sql.DataSource. However, that
is incorrect as according to the 'Common Annotations for the Java™ Platform™' spec:

"In the example above the effective name is com.example.class/myDB and the effective type
is javax.sql.DataSource.class."

If the name is not specified, the generated name should be qualified with the class name of
the instance.
