There's nothing special about using a proxy in front of CouchDB that  
with any other http server.

the proxy's docs are usualy exhaustive for this default case:

http://wiki.codemongers.com/NginxLoadBalanceExample

for varnish just put in couchdb's ip and port into the default config  
file as a backend.
