I didn't spend much time optimizing the _all_dbs pagination because as Newson mentioned in
the other thread, it's doing a file system scan and returning all the items. So if you have
tens of thousands of databases, let alone millions, you're going to be hosed either way. Although
admittedly, making an additional request per database doesn't help matters. When BigCouch
is merged, the list of databases will be represented in a database, and we will be able to
do the standard pagination queries on _all_dbs.
