I was not able to reproduce it but one reason the doc could not be loaded from the btree (when no deletion happen) could be due a stalled btree state. While the index is refreshed, the btree state is too old to contain the information about the attached document. 

Here is a patch to make sure we are always using the latest db state before querying it for the doc.
