A remark from davisp on #couchdb IRC made we wondering why in

    Doc = case extract_header_rev(Req, couch_httpd:qs_value(Req, "rev")) of
        missing_rev -> % make the new doc
        Rev ->
            case couch_db:open_doc_revs(Db, DocId, [Rev], []) of
            {ok, [{ok, Doc0}]}  -> Doc0;
            {ok, [Error]}       -> throw(Error)

couch_db:open_doc_revs/4  is used instead of couch_db:open_doc/2.

I suggest that, if nobody can tell a reason for using open_doc_revs/4 instead of open_doc/2,
the patch should be changed accordingly.

Anyway, I think that this deserves a closer look.
