I've made a lot of fixes and added Futon-based unit tests. I think the code is worth reviewing
for inclusion on trunk now. 

FYI: The test suite works fine in Firefox but fails in Chrome, it seems Chrome does inappropriate
caching (I see 304 from CouchDB in Chrome but 206 in Firefox), so I'm not sure it's my bug.
Performing curl requests matches both expectations (and, hopefully, the spec!) and the results
that Firefox produces.
