Hello,

Paul, I took some time to go further from couch_httpd/couch_httpd_db and explore couch_db
 and couch_stream as well.
This patch has a completely different approach from the previous ones, as it checks only for
the integrity of incoming attachments' requests, including the Content-MD5 field either in
the http header or in the footer when having chunked http encodings.

I also added an Etap test case.

Can you provide some feedback on it?
