[~bhwatts], I've tried to reproduce this error using your sample project w/o success. Sporadically,
I'm getting an error after audio file is being recorded: 'A media source cannot go from the
stopped state to the paused state.', not sure what's causing it, but anyway, the audio file
is created and I can upload it.  Also, it seems to me that you forgot to add device capability
to access microphone (w/o this capability I get the error: 'Access denied').

Could you please confirm that you don't get any information in error callback except for code?
Does this error reproduce if you run application w/o VS2015?
