Phil Steitz resolved DBCP-427.

    Resolution: Fixed

Modified version of the patch committed in r1655301. 

Modifications were to make the behavior configurable.  I added fastFailValidation property
to turn on/off the failfast validation and I also added support for user-supplied sets of
fatal disconnection SQL_STATE codes.
