I reckon that if the proxy server does not enforce proxy authentication, then the 'pu' (proxy
user) and 'pp' (proxy password) can be left out:

(1) USER [pu] PASS [pp] OPEN USER u PASS p
(2) USER [pu] PASS [pp] USER PASS p
(3) USER [pu] PASS [pp] SITE USER u PASS p

Mine, however, DOES enforce proxy authentication.
