I've fixed the first part of this, such that if FileUpload.getHeaderEncoding()
returns null, it will use the request encoding, if there is one, getting this
from ServletRequest or ActionRequest as appropriate.

The second part of this would cause interface breakage, in order to provide a
way to pass the encoding to the FileItem instances. Although FileUpload 1.1 has
not been released, there are so many people using it at this point that I'm
reluctant to make such changes at this time. Feel free to file a separate
enhancement request for this, and mark it LATER, if you wish.
