Updated patch for FileChangedReloadingStrategy plus unit test

Patch updated to incorporate changes suggested by Jorge Ferrer.

FileChangedReloadingStrategy will now correctly update its lastChanged field,
so that hasChanged() need not be called every time reloadingRequired() is
invoked.

I also included the proposed functionality that if a configuration file is
loaded from a jar, the jar file itself should be the target for the strategy.
However IMO the containerFileURL() method seems to be very specific to the
reloading strategy, so I placed the code directly in
FileChangedReloadingStrategy. If we later find out that this stuff is generally
useful, we can still move it to ConfigurationUtils and make it public.
