[fileupload] setSizeThreshold not working as described in documentation

I expected the setSizeThreshold option to work on the file size and not the 
request size as the doc says "Sets the size threshold beyond which _files_ are 
written directly to disk.".

However, I think a better approach would be to have two thresholds :
1. setRequestSizeThreshold : if the request size is greater than x bytes, save 
every file to disk (could be disabled by specifying a size <= 0 for example)
2. setFileSizeThreshold : if the size of an individual file is greater then the 
threshold, save the file to a temp file (again disabled if <= 0)

If both sizes are specified, rule 1 is evalueted first. If the request size is 
less than the threshold, rule 2 is evaluated on a file per file basis.
