Very Strange! From the stack-trace -

UploadAction.java:127 => String contentType = file.getContentType();
where, file.getFileName() has already been called (i.e. file is not null). It 
is an object of the CommonsMultipartRequestHandler$CommonsFormFile class.

CommonsFormFile.getContentType() => return fileItem.getContentType();
where, fileItem.getFileName() has already been called (i.e. fileItem is not 
null). It is an object of the DefaultFileItem class.

DefaultFileItem.getContentType() => return contentType;
where, contentType is a member variable.

The NPE should not be thrown...?!?

But, if the reporter is confident that the problem is in MultipartInputStream, 
it should be reported against commons-fileupload.
