Per popular request and my own enlightened self-interest, I've committed
Michael's re-implementation of SequencedHashMap.  It looks good to me, but I
encourage all interested parties to review it.  It does pass all existing
unit tests.

I plan to re-implement LRUMap as a subclass of SequencedHashMap sometime
this week, unless anyone has issues with including the SequencedHashMap
methods within the contract of LRUMap.
