Hi,

Try something like this:

<map:match pattern="url-form-sends-to.xml">
   <map:generate type="stream">
      <map:parameter name="form-name" value="myFormFieldName"/>
   </map:generate>
   <map:transform src="my-xslt.xsl"/>
   <map:serialize type="xml"/>
</map:match>

I think you're looking for the "stream" generator.
