Or if you change the case of your XSLt files:

       <map:match pattern="form_*.xml">
         <map:generate src="xml/application.xml"/>
         <map:transform src="xsl/xmlToForm{1}.xsl" label="content">
         </map:transform>
         <map:serialize type="xml"/>
       </map:match>
