I wonder if you don't need to put an explicit closing tag
&lt;/script&gt; instead of an empty element.<br>

Put a blank inside to be sure to obtain the following :<br>

to answer your question in the tutorial:

The form only works when the page is referenced as "resource/forms-lib.js
because of the forms-styling.xsl. In there, you can find the following
template:

&lt;!-- Location of the resources directory, where JS libs and icons are
stored --&gt;

  &lt;xsl:param name="resources-uri"&gt;resources&lt;/xsl:param&gt;


  &lt;xsl:template match="head" mode="forms-field"&gt;
    &lt;script src="{$resources-uri}/forms-lib.js" type="text/javascript"/&gt;
    &lt;link rel="stylesheet" type="text/css" href="main.css"/&gt;
  &lt;/xsl:template&gt;

As you can see, the parameter ressources-uri has the value 'ressources'.
If you want to use an alternative location, you have to change it here.
I hope that answers your question (I needed about 2 hours to find it, the
first time I tried to understand the car/country-example).
