AFAICS the namespace is somewhere introduced in the TraxTransformer's
endDocument() method. I don't know why, because with the
JXTemplateGenerator it causes problems, not with the file generator.

I replaced the Xalan transformer with the Saxon transformer, and all my
problems are gone again (Saxon 8.1.1)!

Let me know if this fixes your problem.
