Indeed. The actions default to "attributes" which was the short name
for the request attributes output module. That has been recently
renamed request-attr. Please add
to the action's declaration in sitemap. E.g.

	  <map:action name="my-dbadd" src="org.apache.cocoon.acting.modular.DatabaseAddAction">
