XInclude is implemented as a processor.  Try adding this line to your XML
file (e.g., above the xslt processing instruction):
<?cocoon-process type="xinclude"?>

Also, I think I had to use the xinclude namespace on the attributes to make it work, like
<xinclude:include xinclude:parse="xml" xinclude:href="...."/>

I was trying something like this:

<paragraph xmlns:xinclude="http://www.w3.org/1999/XML/xinclude">
         <xinclude:include href="hello-disclaimer.xml"/>
</paragraph>

But nothing happend :(
Does cocoon recognize the "http://www.w3.org/1999/XML/xinclude" namespace?
