Make sure your samples directory is within the cocoon
subdirectory:

I.e. it should be:
http://localhost:8080/cocoon/samples/fo/test-fo.xml
and NOT http://localhost:8080/samples/fo/test-fo.xml

Can you access:
http://localhost:8080/cocoon/samples/index.xml ? What
happens when you access one of the FO links within
index.xml?
