Maybe one of your xsl files is dropping the cocoon-format processing 
instruction. Check that all xsl files have something like

<xsl:template match="processing-instruction()">
  <xsl:copy-of select="."/>
</xsl:template>

That means it is DEFINITELY not outputting valid XML.
